What's Happening
Excessive moisture retention in the substrate, often caused by 'double potting' (cache pots trapping water) or constant misting of moss poles that keeps the root ball perennially wet. This leads to root oxygen deprivation and Pythium/Fusarium rot.
How to Fix It

Check the roots: Remove the plant from the pot; if roots are black and mushy, it's rot.

Root Surgery: Trim away all rotting roots and soak the remaining healthy roots in a diluted hydrogen peroxide solution (1 part 3% H2O2 to 10 parts water).

Repot in Airy Mix: Use a chunky aroid mix (50% bark/perlite) to ensure maximum drainage.

Dump the Cache: Never let the plant sit in standing water in a decorative outer pot.
How to Prevent It
Allow the top 2-3 inches of soil to dry completely before watering. Focus on ambient humidity (>60%) rather than direct leaf/pole misting.
